First Visit
What Will Happen On My First Visit?
Dr. Adornato and her staff will tailor your first visit to our office according to your specific needs. Some new patients require attention to an isolated problem, while others require a more comprehensive evaluation. Your level of care will be determined at this first visit by meeting with Dr. Adornato for your examination. Information concerning your case will be collected. Our office normally requires a comprehensive medical and dental history, clinical examination and radiographic analysis. Dr. Adornato will study your clinical information and develop a treatment plan that will attend to your specific needs. That treatment plan will be thoroughly discussed with you and your general dentist.
Please assist us at the time of your initial visit to our office by providing the following information:
-
Your referral slip and x-rays from your referring dentist, if they were provided to you.
-
A list of medications you are currently taking (please note that we appreciate you filling out your 3 patient history forms that will be mailed to you prior to your visit.)
-
If you have dental insurance, please bring your dental insurance information or dental insurance card with you to the appointment.
Please Note: All patients under 18 must be accompanied by a parent or guardian at ALL appointments.
Please notify the office if you have any medical conditions that may impact your care: (e.g. artificial heart valves or joints that may require antibiotic pre-medication or other conditions that may also require additional attention).
Our clinical examination will evaluate your medical and dental health history. A thorough periodontal examination will be completed to evaluate bone loss due to infection, loose teeth, lost teeth, TMJ and bite abnormalities. An oral cancer screening is also completed as part of your evaluation and review for other signs of periodontal disease.
